For more than 55 years, ConnextCare has been providing family-oriented healthcare services to residents of Oswego County and surrounding areas. ConnextCare offers a wide range of comprehensive healthcare options – including primary care, mental health, dental health, substance abuse treatment, and telehealth. It operates nine school-based health centers and accepts all major insurances. A sliding fee scale discount program is also available for eligible patients. In addition to its main location in Pulaski, ConnextCare has offices in Central Square, Fulton, Mexico, Oswego, and Phoenix.

WALK-IN CLINICS & ACUTE CARE SERVICES

ConnextCare’s Walk-In Clinic in Pulaski provides fast, convenient care for sudden illnesses and minor injuries. No appointment needed, and no requirement to be an existing patient. The clinic is staffed by skilled healthcare professionals who offer compassionate, efficient treatment for a wide range of non-emergency conditions. On-site lab and diagnostic services are available and the acute care services are offered with extended hours to better serve busy families and working individuals.

All other ConnextCare locations offer acute care and/or walk-in hours for established patients. All major insurances are accepted, and a sliding fee scale is available for eligible patients.

Syracuse Community Health provides primary care for adults and pediatric care for kids up to age 21. General medical services include annual physicals; lab work; nutrition counseling; diagnostic x-rays; mammography; ultrasound; cancer screenings; vaccinations; and COVID-19 testing. Pediatric care services include routine checkups and wellness exams, well-child visits, vaccinations, sick visits (same-day or next-day appointments), developmental vision and hearing screenings, milestone tracking, school sports and college physicals, nutritional consultations, ADD and ADHD behavioral screenings, depression screenings and referrals to mental health services, and support for breastfeeding mothers.
