Catholic Charities of Onondaga County provides mental health programs, including a counseling program that provides outpatient individual, couple, family, and group psychotherapy services for those who need accessible and affordable mental health services, with a focus on helping the most challenged in the community, including those in poverty, refugees, elderly, and those who have experienced trauma. Its Hope Connections is a peer-led recovery center and a gateway to wellness for people affected by mental illness, substance use, or addiction. Cayuga County Community Mental Health Center provides mental health care and treatment to residents of Cayuga County. Those interested in starting services can walk in to the clinic between 8:30 a.m. and 3:30 p.m. Monday through Friday. Services will start on the same day. Center for Mental Health & Wellness for Youth includes child/adolescent psychiatrists, psychologists, counselors, and nurses who work closely with families so they can successfully support their children during and after treatment. For greater than 55 years, ConnextCare has served the residents of Oswego County and surrounding areas with compassionate, patient-centered care. Its Mental Health Services provide individuals and families with comprehensive behavioral healthcare in a supportive and confidential setting. The professional mental health team includes adult and pediatric psychiatrists, psychiatric nurse practitioners, licensed clinical social workers and licensed mental health counselors who work collaboratively with primary care providers to ensure integrated, whole-person care. Services include individual and family therapy, crisis intervention, medication management, and coordination of care with other community resources. ConnextCare’s Mental Health Services are available at its main site in Pulaski and at offices in Central Square, Fulton, Mexico, Oswego, and Phoenix. Helio Health is a comprehensive substance use disorders and mental health disorders services provider. Among its programs are drug and alcohol withdrawal and stabilization services, detoxification, inpatient rehabilitation, child and adolescent services and problem gambling counseling, opioid treatment program, as well as counseling for mental health disorders such as depression. It also provides outreach services, community residences, a training institute, peer support and engagement, and a crisis center.

The Crisis Bed Program is available for members of the community in need of short term (7-14 days) crisis support in order to avert unnecessary psychiatric hospitalization, prevent a lengthy hospitalization and/or provide step down support following hospitalization prior to return to independent living status, or while awaiting alternative community placement.

The Family Care Program is a certified residential program for adults recovering from a mental illness. Family Care provides support and supervision for individuals who need a minimum level of structure and supervision to achieve and/or maintain improved stability with community living. Family Care provides an opportunity to live in a private family home in the community and receive support and guidance of a caring family. Residents are carefully screened and matched with private citizens willing to share their homes to enhance the quality of life for the resident. The staff of Family Care works closely with family care providers to ensure a positive experience for both the resident and the provider. The length of stay in the residential program is voluntary and dependent on the residents’ choice and need.

Supervised Residence and Apartment Treatment Programs: The program provides residential services to psychiatrically disabled adults, allowing those diagnosed with a severe and persistent mental illness to live in the community in the least restrictive environment. The program offers 2 levels of residential and rehabilitative assistance to adults who cannot live in the community independently. Certified residential services are available at two levels of supervision.

As a behavioral health services inpatient and outpatient provider in Oswego County, this Oswego Health facility provides provide specialized care for patients aged 18 and up. This center features 32 inpatient beds. Patients will find a welcoming, soothing, and healing environment that includes secure outdoor spaces, comfortable interior areas, and a kitchen area. In addition, primary care services are available onsite to assist those that utilize these services become healthier overall.

• Oswego Health — Assertive Community Treatment (ACT) Team

The Assertive Community Treatment (ACT) Team is a mobile, multidisciplinary, mental health team through Oswego Health that brings support services and delivers comprehensive, person-centered treatment and rehabilitation to individuals over the age of 18 with severe and persistent mental illness. ACT Team services include help and support 24 hours a day, 7 days a week. As a multidisciplinary team, it provides the treatment and services needed; helps individuals in their homes, neighborhoods, workplaces, and other places where problems arise; and works closely with each client to develop an individualized plan that is reviewed daily by all involved staff members.
