The senior day habilitation program is designed to provide adults older than 55 with a disability with age-appropriate services, including activities to help preserve mobility and cognitive skills to promote independence.

• Catholic Charities Elderly Services

The program works with adults aged 60 and older, assisting them to remain safe and comfortable in their homes for as long as possible.

Services include: Area North Transportation Services (ANTS), which provides transportation for clients in Northern Onondaga County; Expanded In-home Services for the Elderly Program (EISEP), which provides linkage to support and services, including in-home assistance; Project Fix, which offers minor household repairs, including ramp construction; Neighborhood Advisors, which provide links to community programs, help with finances, and more. The Elderly Services program also works with the Onondaga County Office for Aging and Adult Protective Services to provide direct advocacy and assistance to those who are identified and referred.

Westcott Community Center offers comprehensive programs and services for older adults 60 plus. Breakfast, lunch and daily activities are offered, and there is also a senior transportation program which provides older adults 60+ with rides to and from the center, medical appointments, grocery store, pharmacy, etc.

• Enhance Fitness

Enhance Fitness is a nationally-recognized, proven and community-based senior fitness and arthritis management program designed to help older adults become more active, energized, and empowered for independent living. Participants join in low-impact exercise classes that are safe yet challenging and work to increase cardiovascular endurance, strength, balance, and flexibility- all of which can reduce arthritis symptoms. Must be a Y member.

Through the New York State Golden Park Program, residents 62 and older have free vehicle access to most parks, boat launches sites and arboretums and reduced fees to golf courses and historic sites on weekdays, except holidays. No application is required as a NY State driver’s license or non-driver ID card serves as the pass. Among the programs offered through the agency’s Center of Healthy Aging are The One to One Program, which matches nursing home residents with volunteers to spark joy and social connection, and the Senior Companion Programs, in which volunteers aged 55 and over provide assistance and friendship to adults who have difficulty with daily living tasks. The program helps these adults remain independent in their homes. Senior companions give families or professional caregivers time off from their duties and provide friendship for their clients. Loretto is a comprehensive continuing healthcare organization which provides a variety of services for older adults throughout Central New York. Loretto transforms eldercare in Central New York by deinstitutionalizing nursing homes and long-term care services and replacing them with home-like settings utilizing person-first care. • Memory Care Corral

The Memory Care Corral is a customized program for individuals living with dementia and their caregivers. Guided sessions with horses provide both the individual with memory loss and their loved one with therapeutic, non-riding activities. These 90-minute sessions reduce stress and enable participants to better communicate and support each other. No horse handling experience is necessary. Sessions for 2026 begin in the spring. There is a small fee for participation. Funded through The Athelda Fund.

Nascentia Health is one of the most trusted and experienced home-and community-based care agencies in New York. Headquartered in Syracuse since 1890, the organization focuses on coordinated care and services that help people stay safe and healthy in their own homes. Nascentia Health provides in-home skilled nursing and therapies for patients of all ages and home health aides for people who need professional caregivers at home. Nascentia Health Options is a managed long-term care plan for individuals on Medicaid who are chronically ill or disabled and need services to allow them to remain at home. Nascentia Health Plus Medicare Advantage Plan is a dual special needs plan (DSNP) that offers additional benefits to individuals who are eligible for both Medicare and Medicaid.

• Neighborhood Advisor Program (NAP)

Neighborhood Advisors assist older adults in obtaining the services they need to remain independent in their homes. The services of neighborhood advisors are free and confidential. An appointment is needed to meet with a neighborhood advisor, and the meeting can take place in the senior’s home or at a local library, senior center or community center. Neighborhood advisors assist with the completion of medical applications and other forms; assist in locating services related to health care, legal advice, transportation, housing, nutrition and exercise, socialization, and caregiver information; and provide information about income-stretching programs. Participating sites include Canton Woods Senior Center, Baldwinsville; Catholic Charities, Liverpool; Dunbar Association, Syracuse; Huntington Family Center, Syracuse; InterFaith Works, New American/Refugee Seniors; SAGE Upstate, LGBTQ+ Seniors; Salvation Army, Syracuse Northeast Community Center and Westcott Community Center, Syracuse.

The department provides support to improve the quality of life and overall well-being for adults and those with long-term care needs. Among its divisions are Aging Services; Adult Protective Services; Long-Term Care Services; NY Connects; Senior Resource Center; and Emergency Preparedness for Persons with Disabilities.

The Office for Aging offers services both in the community and in the homes of seniors who live in Onondaga County; is a focal point for information related to healthy living and/or care in the home; helps seniors and their caregivers to get the services they need; and provides services directly as well as through partnerships with local community-based organizations.

The Oswego County Office for the Aging directly provides, or contracts to provide, a number of services to enable older residents of Oswego County to remain within their home safely and with dignity. These caregiver services including respite; training and support groups; health insurance counseling and assistance; nutrition services which includes home delivered and congregate meals, nutrition counselling & nutrition education; senior in-home care and assistance; health program; transportation to and from senior congregate dining sites, including a shopping day; transportation to out of County doctors’ appointments; animatronic pets; application assistance (SNAP, Medicaid, housing); an annual senior picnic; and the friendly call program.

 PACE CNY (Program for All Inclusive Care for the Elderly in Central New York) provides a continuum of acute and long-term care services for seniors 55 and older in Onondaga County who are eligible for Medicare and Medicaid. This program is managed by an interdisciplinary care team and is based on the national PACE model of managed care for older individuals who qualify for nursing home care but want to remain living at home. All PACE CNY participants must agree to receive medical care from the PACE CNY provider network.

The information & assistance case manager can help provide information, referral services and advocacy to seniors 60 years or older who are residing in Onondaga County, as well as for their families/caregivers. The Neighborhood Advisor assists seniors in Onondaga county who are 60+ years old in obtaining a variety of services needed to remain living independently in their community.
